Driveway Culvert Permit Request

Please complete the fields below and click the "Submit" button at the bottom.

Staff will check submitted applications each weekday and will call you with the cost of the permit and ask how you want to pay for it. Permits may be paid with a credit card over the phone or you may mail a check to City of Medford, Engineering Division, 200 S. Ivy Street, Medford, OR 97501. Once payment has been received, the permit can be faxed, emailed, or mailed to you. If you have questions please call Public Works Engineering & Development (541) 774-2100.

All driveway culverts shall be installed by City crews. See Medford Municipal Code 3.360.

*Note: You may fax a drawing to (541) 774-2552 showing your street frontage and the desired location of the culvert. Culverts are usually sold in 14 foot lengths. Twenty-eight feet of culvert is used for a standard residential driveway.

You must have a permit to perform any type of work in the public right-of-way. Once you have obtained your permit, you will need to mark the desired location of where the culvert is to be installed. You may pick up two wooden stakes, free of charge, in the Engineering Division of Public Works at 200 S. Ivy Street, second floor, or use your own stakes.

After City crews place the culvert and cover it with rock, it is the responsibility of the applicant/owner to hire a contractor to pave the driveway from the property line to the existing asphalt pavement in the street. Per Medford Municipal Code 10.746, all parking and vehicle maneuvering areas must be paved. Paving in the right-of-way shall be performed by a contractor that is prequalfied to work in the public right-of-way. You may check the Public Works' list of prequalified contractors by clicking here. Call (541) 774-2100 for an inspection of the paving in the right-of-way.

Contractors are required to have a current Construction Contractor's Board (CCB) license. You can search for a contractor's CCB license at http://www.ccb.state.or.us/ then select "Contractor Search". Contractors are also required to have a current City of Medford Business License. You can search for a contractor's business license from the City of Medford homepage. Select "Online Services/Business License Online Services/License Information".

A property owner may pave on their private property and no permit is required to do so.
