Driveway Apron Repair Permit Request

Please complete the fields below and click the "Submit" button at the bottom.

Staff will check submitted applications each weekday and will call you with the cost of the permit and ask how you want to pay for it. Permits may be paid with a credit card over the phone or you may mail a check to City of Medford, Engineering Division, 200 S. Ivy Street, Medford, OR 97501. Once payment has been received, the permit can be faxed, emailed, or mailed to you.

Permits are required in order to perform any type of work in the public right-of-way.

Standard driveway apron width is currently 18 feet plus a 6 foot flare on each side. See Medford Municipal Code 10.550 Access Standards.

Contractors must be prequalified to work in the public right-of-way. You may check the Public Works' list of prequalified concrete contractors by clicking here.

Contractors are required to have a current Construction Contractor's Board (CCB) license. You can search for a contractor's CCB license at http://www.ccb.state.or.us/ then select "Contractor Search". Contractors are also required to have a current City of Medford Business License. You can search for a contractor's business license from the City of Medford homepage. Select "Online Services/Business License Online Services/License Information".

The property owner is allowed to repair their own driveway apron if they are doing the work themselves.
